Thomas Gleixner c9317202af x86/entry/64: Use native swapgs in asm_load_gs_index()
When PARAVIRT_XXL is in use, then load_gs_index() uses xen_load_gs_index()
and asm_load_gs_index() is unused.

It's therefore pointless to use the paravirtualized SWAPGS implementation
in asm_load_gs_index(). Switch it to a plain swapgs.

In order to build the documentation, use ``make htmldocs`` or
``make pdfdocs``.  The formatted documentation can also be read online at:

There are various text files in the Documentation/ subdirectory,
several of them using the Restructured Text markup notation.

Please read the Documentation/process/changes.rst file, as it contains the
requirements for building and running the kernel, and information about
the problems which may result by upgrading your kernel.
