Remove the old struct urb and struct usb_ctrlrequest structures.
Remove fMP_CONTROL_READS flag.

PIPEnsControlOut and PIPEnsControlIn are nolonger
atomic but they do need a usb lock.
Add new mutex lock for these functions and PIPEnsControlOutAsyn.
