* render(toc): render expandable toc toggle

* ui(toc): js/css logic to toggle toc

* test: update rendered output css selector

* config: add `html.fold.[enable|level]`

* renderer: fold according to configs

* doc: add `output.html.fold`

* refactor: tidy fold config

- Derive default for `Fold`.
- Use `is_empty` instead of checking the length of chapters.
