Dan Williams dfddb969ed iommu/vt-d: Switch from ioremap_cache to memremap ...

In preparation for deprecating ioremap_cache() convert its usage in
intel-iommu to memremap.  This also eliminates the mishandling of the
__iomem annotation in the implementation.
