Alexey Dobriyan ee1e6ab605 proc: fix /proc/*/pagemap some more 
struct pagemap_walk was placed on stack, some hooks are initialized, the
rest (->pgd_entry, ->pud_entry, ->pte_entry) are valid but junk.
