Kevin Cernekee 0dc54e9f33 mtd: add MEMERASE64 ioctl for >4GiB devices ...

New MEMERASE/MEMREADOOB/MEMWRITEOOB ioctls are needed in order to support
64-bit offsets into large NAND flash devices.
